Patient Access
The Surgery offers a service called patient access. This allows you to order repeat medication without needing to call or come into the surgery!
To sign up to patient access, please call into surgery and collect a consent form with some form of I.D and we our reception team can give you your unique log in details to be able to access this service.
Patient Access also allows you to nominate your pharmacy of choice which means you don’t need to collect your prescription from the surgery, you can go straight to your chemist!
Please return the completed form to the practice in person with 1 form of ID, e.g. passport, driving licence, Armed Forces ID, utility bill, bank statement, mobile phone bill etc.
To ensure confidentiality we are only able to accept registrations in person – i.e. you cannot give your details to anyone else to register for you. Our reception staff will register you and provide you with unique login details instructions for the website.
